Two things are required to enter your local award winners into the running for our national-level award: the completion of this reporting form and the supporting information collected as part of the nomination form. If your district didn't use our standard forms to collect nominations, please find the appropriate nominating form
here to submit those additional details (you or the nominating coach can do this).
